Home Care Package Onboarding
The Home Care Package Onboarding is the process of registering a client with My Aged Care. We’ll help you submit all the necessary documents and assessments that are required when signing up with a Home Care Package provider.

Aged care services in Australia are governed by My Aged Care. To receive a Home Care Package, My Aged Care is the primary contact to begin your assessments. To start your Home Care journey, contact us to find out how we can help you.
My Aged Care states “An older person who needs coordinated services to help them stay at home for longer” to satisfy the requirements for Home Care Package funding.
- Other requirements include being 65 years old (50 years or older for Aboriginal and Torres Straight Islander People).
- Noticed a change in what you can do or remember.
- Have been diagnosed with a medical condition or reduced mobility.
- Experienced a change in family care arrangements.
- Experienced a recent fall or hospital admission.
The Aged Care Assessment Team (ACAT) of the Australian Government will carry out a thorough evaluation of your health, lifestyle, and circumstances to determine areas where you require assistance with daily activities. This assessment will help recommend the appropriate services that align with your needs.
After your assessment you will receive a letter advising that you have been approved for a Home Care Package. There may then be a waiting period before the Home Care Package is assigned to you. Once the Home Care Package is assigned, you will receive a referral code, this needs to be given to a Home Care Package Provider before you can start receiving services.
There are two options when managing your Home Care Package. Fully managed or self-managed. Fully managed home care is the traditional method of care, and often comes with high rates. Self-managed home care is the new home care option giving power to senior Australians to self-direct their package and make their own choices in how they receive care.
For those that want the flexibility and lower costs that come with self-managing a Home Care Package, but require extra assistance with finding support workers, Absolute Care For You provides coordination of your sevices. This helps you with choosing and managing your carers in your area that best suit you.
Trilogy Care a Home Care Package (HCP) provider partnered with Absolute Care For You. As a HCP Provider, Trilogy Care is responsible for the management of HCP funds and care management of HCP recipients.
